Registered Nurse (RN) Case Manager – Home Health & Hospice
Location: Rexburg, ID (In-Person)
Schedule: Full-Time, Monday–Friday, 9:00 AM – 5:00 PM (With occasional weeknight and weekend on-call rotation)

Compensation & Financials:
- Hourly Pay: $30.00 – $40.00 per hour (Based on experience).
- Guaranteed Hours: Paid by the hour—not by the visit—with a guaranteed 40 hours per week!
Key Responsibilities:
- Case Management: Manage and coordinate comprehensive patient care for home health and hospice caseloads.
- Care Planning: Perform clinical patient assessments and develop individualized, patient-centered care plans.
- Collaboration: Partner with physicians, families, and interdisciplinary team members to ensure seamless care.
- Documentation: Maintain accurate, timely clinical documentation within the company’s EMR system in compliance with federal regulations.
Qualifications & Requirements:
- Licensure: Current Registered Nurse (RN) license in the state of Idaho.
- Experience: 1–2 years of nursing experience is preferred, but not required (Home health or hospice experience is a major plus!).
- Skills: Strong clinical, organizational, and independent workload management skills.
- Travel: Must have a valid driver’s license and reliable transportation for local travel.
